What is a "Red Fail" fault for a signal monitor?

What is Red Fail?
The monitor detects a channel which has no active color output to the field, aka a dark approach).
When you get a Red Fail, the issue is in the cabinet. From the top of the Terminal block up in the cabinet wiring and signal components.

The term Red Fail is when the intersections would rest in all red and a vehicle would come to an approach. When the lights were transitioning to green in that approach and a fault occurred, they would say if it is not all red then there is a βRed Fail".
